Admissions Process

How to Apply:  

Rainbow Acres has a two-part admissions process to assess the candidate’s physical, mental, and social health, interests and personality. 

Part 1: 

Admission Inquiry: 

The Admission Inquiry is basic information gathered from a family member or guardian about the candidate. Please click on the link to complete the form online and submit electronically, or print, complete and return by mail to:

Part 2:

Full application package: 

After submission of the Admission Inquiry form, a member of our Admissions team will contact the parent or guardian of the candidate. Upon request, Rainbow Acres will send a full application package for a family member or guardian to complete. The full application package helps us learn more in-depth about the candidate, covering topics such as family background and health history, as well as social and cognitive abilities. 

Admissions Process:

Rainbow Acres will review the completed application information. Qualified candidates and their family members or guardians will be invited to meet with our Admissions Team for an interview conversation. We wish to meet you in person and talk about mutual goals and desires for living at Rainbow Acres, and to ensure a good fit for both the candidate and for Rainbow Acres.
